Where is the Foundation office located?

The Foundation's main office is located at 2 Queen St. E., 7th Floor, Suite 712 (view map). There's just one exception. If you're looking for the Gift Planning team (Cynthia Collantes and Sandra Smith), they are located in the hospital at 30 Bond St., Donnelly Wing, 1st floor, Room 1-021.

If you are donating by cheque, please pay to the order "St. Michael's Hospital Foundation". If you're donating by credit card, you can use our Print-and-Mail Donation Form. You may mail your donation:

You may call the Foundation at 416.864.5000 to make your donation by phone or to ask a question. Our office hours are Monday to Friday from 8:30 a.m. - 4:30 p.m. Please note, our offices are closed on statutory Canadian holidays.

DONATIONS IN MEMORY/HONOUR/THANKS

How do I make a donation in memory/honour/thanks of someone?

Our online donation form has a series of questions which tells us what type of gift you wish to make. As you answer each question, the next one is revealed. The 3rd question asks, "Is your donation in MEMORY or in HONOUR or in THANKS?" If you select YES, three buttons appear on the form, allowing you to choose what type of tribute donation you wish to make: "In Memory", "In Honour" and "In Thanks". Clicking any one of those buttons will open a space beside it where you can type in the name of the person on whose behalf you are donating. If you have an email address for someone you'd like to make aware of your donation, you'll can send an eCard with a personal note. If not, you can use the COMMENTS box (above the Payment Details section) to enter a personal message and ask that it be sent along. You can also call 416.864.5000 to provide this information and make your donation over the phone, or use the Print and Mail donation form and complete the "Tribute" section at the top of page 2.

I made a donation in memory/honour/thanks of someone. How will they know about it?

Whether you donate online, by telephone or by mail, if you've told us your donation is in memory, in honour or in thanks, your gift of $20 or more will be acknowledged with a tribute card sent to the individual(s) informing them of your thoughtfulness.

DONATIONS TO SPECIFIC PROGRAMS OR CLINICS

When I donate online, how can I make sure my gift goes to a specific program or clinic?

If you donate online you can use the COMMENTS box to specify the doctor, clinic, program or campaign you'd like to support. Our finance team will credit your donation to the fund you specified. You can also call 416.864.5000 to provide this information and make your donation over the phone, or use the Print and Mail donation form and complete the "Gift Designation" section at the bottom of page 1.

ONLINE DONATIONS AND RECEIPTS

I donated online but I haven't received my tax receipt. What should I do?

If your donation was PERSONAL (not corporate) and you reached the confirmation page saying your receipt was emailed to you, sometimes there's a delay of up to one hour before that email reaches your inbox. Be sure to check your spam or junk mail folders in case it landed there. If, after one hour, you haven't received it, or if your donation was CORPORATE, please call 416.864.5000. We'll check that your donation was received and if so, we'll send you a copy of your tax receipt.

My tax receipt was not attached to the email I received thanking me for my donation. What should I do?

Your tax receipt is embedded at the very bottom of the email we sent. Scroll down past the St. Michael's Foundation signature, the "Blackbaud" logo and "nonprofit software" link. You should see it there, but if you still have trouble, please call 416.864.5000 and we'll send you a copy of your tax receipt.

I filled in my credit card information and clicked "process my gift" but nothing has happened since. Was my card charged?

We successfully process a number of online transactions every day, but occasionally we hear from people who have this experience. In each case the card was not charged. If you do not reach the confirmation page in 20 seconds or less, you should close/quit your web browser, then delete your history and clear your cache before trying again. We realize your time is valuable, so you can call us at 416.864.5000 and make your donation over the phone instead. We will make sure that your card isn't charged twice.

I filled in the form, clicked "process my gift" and it says there are errors on the page, but I've corrected them. Why can't I continue?

People using the autofill feature or the Internet Explorer browser have reported this experience. Delete your history and cookies, disable autofill if you're using it, clear your cache and close/quit your web browser before trying again. If you have another browser, try using it instead. Or, call us at 416.864.5000 and make your donation over the phone using your credit card instead. We will make sure that your card isn't charged twice.
